Student Summer Program Forms Packet Page

In order to complete your child’s registration for the Spoleto Study Abroad Summer Program, the forms packet below must be completed and submitted by May 31, 2025. Note: you will be able to save your form and resume at a later time.

Forms include:

• Statement of Understanding

• Rules Contract

• Photo/Video Release Form

• Health Insurance Verification Form

• Medication & Pre-Existing Conditions Info Form

• Post-Conditional Acceptance Health Examination (Forms I & II)

  • Form II must be completed by a licensed physician. You can download the form here. Completed forms can either be uploaded at the end of the forms packet or by emailing it directly to summerprograms@spoletostudyabroad.org.
